ABOUT US
Findlay Family Practice is a private primary care office that is located on the south end of Findlay. It was established in 1996 by Dr. Frank Cosiano and Dr. Robert McEvoy. Since opening, our practice has housed 15+ providers. From MDs and DOs to Physician Assistants and Nurse Practitioners. We also have several medical assistants, phlebotomists, an RN and LPN all in house!
We evaluate and treat anything and everything (non emergent); from the flu and ear infections, to sports/work physicals, and chronic conditions such as hypertension, Type 2 Diabetes, COPD, thyroid disease. We also specialize in preventative medicine, being sure our patients are up to date on vaccinations and recommended screenings to ensure longevity and a lifetime of wellness.
Our Staff
Our full time staff consists of over 20 professionals, including a Registered Nurse, Licensed Practical Nurses, Medical Assistants, Phlebotomists, Medical Technicians, Medical Records Technicians, and Telephone triage personnel. They receive continual training to stay current with modern medical equipment and techniques.

Heather Nizohnii Kinsel-Evans, M.D.
Findlay Family Practice welcomes Heather N. Kinsel-Evans MD, to our team of physicians. Dr Kinsel-Evans joins us all the way from Robinson, Illinois!

Clinical interests: Pediatrics, women's health, chronic disease management and preventive care. She enjoys building long term relationships with her patients and providing individuals and families with the tools they need to take control of their lives and health.
Personal interests: Dr. Kinsel-Evans enjoys running, skiing, traveling, listening to live music and sending time with her family.
Jackie Boucher-Stewart, PA-C
“I am excited to be a part of Findlay Family Practice and care for residents within our community. My passion is caring for people and building relationships along the way. I have devoted my entire adult life to caring for those around me. I started my career in medicine as a Physical Therapist Assistant for 13 years before going back to school and becoming a Physician Assistant.”

Experience: Jackie has an extensive background in Pediatrics, Orthopedics, Family Practice, and Urgent Care Medicine.
Clinical interests: Jackie is very passionate about preventative medicine and staying healthy to prevent chronic illness. She enjoys sorting out the mysteries of medicine while providing amazing care along the way.
Personal interests: Jackie enjoys spending time with her family, listening to music, gardening, dancing, exercising, and water sports. She has even been a spin instructor!
Grace Hoerig, MPAS, PA-C

I was born and raised in Findlay, Ohio and am so excited to build my own life here now!”

Clinical interests: Although Grace is a recent graduate, she has a passion for preventative medicine and even has a minor in Public Health. She also really enjoys focusing on her patients’ overall health which includes Mental Health and other psychiatric disorders.
Personal interests: Grace enjoys spending time with her family and friends, cooking, reading, exercising and being outdoors.
